What they needed

Care Continuity helps patients and health systems close the loop on follow-up care—navigating discharge recommendations, scheduling logistics, and in-network provider alignment. Together with Kem Design, I rethought the site's information architecture and collaborated on page design before building the Webflow experience with Mast.

Together with Kem Design, I helped evolve the brand to feel more human—replacing dated illustration with real photography, vibrant gradients, and a ribbon motif for the patient–provider connection. I carried that system into the web build for enterprise buyers and into durable templates for solutions and provider resources.

The problem

The existing site did not match the credibility Care Continuity needs with hospitals, ACOs, and health plans. Dated visuals and unclear structure made a complex product suite harder to trust and harder to parse.

Enterprise healthcare buyers evaluate quickly. A product with 100+ health system customers and 3M+ patients navigated deserved a web presence that communicated authority without losing the human side of care navigation.

What I built

I reworked the IA around who Care Continuity serves—patients, health systems, and providers—and built modular solution pages for ED Connect, Referral IQ, Readmission IQ, and CarePath IQ so each offering tells a clear story without a one-off layout every time.

Mast's 12-column component system kept heroes, proof metrics, CTAs, and content bands composable across templates. A separate resources microsite gives providers extensive information without cluttering the main conversion path.

We shipped the full Webflow build in four weeks—structure, templates, and CMS collections—not a surface-level reskin.
